Fill out the form with your complete name, address, phone number, player code and points. You must include your points and code if you are currently in the book. If you don't know, call your local promoter. We must receive your entry by June 12th. Pre-registration is for package deals only. You will receive a package deal receipt in the mail as you have in the past. This receipt must be presented at the tournament in order to sign up for any doubles event with your partner. All other events, not included in the package deal, you will be able to sign up for on site. Package deal explanations can be found on previous pages. If your membership has expired, you must include your renewal fee with this registration form.

Room Reservation Information

The Hilton Atlanta Airport in Atlanta, Georgia is the host of the 2001 USTSA National Championships.

Please make your reservations as soon as possible. A one-night's room deposit is required to guarantee accommodations. Payment will be accepted by check or major credit card. The room rate is $75 per night for single to quad occupancy per room.

A copy of a Hilton Atlanta Airport room reservation card, seen below, includes all of the details necessary to reserve rooms. We recommend immediately contacting the Hilton Atlanta Airport by telephone as we anticipate our room block to fill up quickly.
